Companion Care / Private Duty Care

Companion care or private duty care primarily provides support services and companionship for seniors who are generally healthy and who want to remain independent and in their own home. It can include a wide range of non-medical services that help make a senior’s life more manageable. These services can include light housekeeping, assistance with activities of daily living (ADLs), medication reminders, transportation to appointments, and more. Companion care is often times provided for seniors in their homes, but can also be provided to those in assisted or independent living facilities.

Shared Care Services

The services that can be provided for under companion care changes based on the senior and their needs. The goal of companion care is to:

Companion care can also include physical care such as grocery shopping, house cleaning, and chores that support or that maintain the independence of a senior or couple. Companion care should enable the senior to enjoy life.

Rehabilitation / Outpatient Therapy

Our Pathways TransitionCare® program incorporates a multi-disciplinary approach to aggressively pursue the goal of rehabilitation. We focus on restoring quality and purpose of daily living for those with a serious illness or injury. Pathways’ clinical specialists support our residents to maximize their recovery and independence. Our Therapy to Home Program® ensures a smooth and successful transition for our clients.

After an injury or illness all you want to do is get back to living your life as quickly as possible. Sometimes this means getting help from a trained professional who is there to work with you to build your strength, flexibility and muscle control. Our Pathways TransitionCare® program incorporates a multi-disciplinary approach to aggressively pursue the goal of rehabilitation. We focus on restoring quality and purpose of daily living for those with a serious illness or injury. Pathways’ clinical specialists support our residents to maximize their recovery and independence. Our Therapy to Home Program® ensures a smooth and successful transition for our clients. Our therapy treatment plans are overseen by a physician and may include:

Physical Therapy:  Physical therapy attempts to address the illnesses or injuries that limit a person’s ability to move and perform functional acts in their daily lives. PTs use an individual’s history and physical examination to arrive at a diagnosis and establish a management plan and, when necessary, incorporate the results of laboratory and imaging studies.

Occupational Therapy: A form of therapy for those recuperating from physical or mental illness that encourages rehabilitation through the performance of activities required in daily life.

Speech Therapy: Training to help people with speech and language problems. A speech therapist will address both the physical as well as cognitive impairments that may impact a person’s speech.

Aquatic Therapy: Treatments and exercises performed in water for relaxation, fitness, physical rehabilitation, and other therapeutic benefit. Typically a qualified aquatic therapist gives constant attendance to a person receiving treatment in a heated therapy pool.

Outpatient Therapy: Often times following an inpatient stay, additional services are required to extend and complement the treatment that was received during a hospital or nursing home stay. Other people may be referred directly from a physician to an outpatient program. In either case, our physical, occupational, and speech therapists can help you build your strength, endurance, flexibility and cognitive functioning to regain your independence.

 




Respite Services

Being a caregiver is an important role; however there are times in which additional support and help may be needed outside of the home. We offer many respite solutions for caregivers throughout our continuum of services. Whether respite is needed for a few days or several weeks, we can accommodate your needs. Our respite option gives caregivers the opportunity to rest and regain energy, with peace of mind knowing their loved one is being cared for, engaged, and active.

Being a caregiver for an aging or ill family member can be enormously rewarding, but it can also be exhausting and emotionally draining. As a caregiver, you feed, transport, clean, entertain, protect, and offer comfort and support. You manage your own busy schedule during the short, sometimes unpredictable lulls in your loved one’s schedule. It is no wonder that almost half of family caregivers rate their job as emotionally stressful.  Despite your very best intentions, caring for an elderly parent can often lead to caregiver burnout, depression, and health problems.

Does homecare require a physician order?

  • Yes. Home care requires an order from a physician. We are happy to work with your physician if you feel homecare is a viable solution for your recovery needs.

What is “skilled care”?

  • Skilled care is care that is provided by a licensed professional.  These are typically nurses, therapists, or a social worker.

What does homecare cost?

  • Homecare is a covered service by most insurance providers. Shared Services Home Healthcare participates with many insurance contracts. However, insurance agreements vary as to specific coverage. For help understanding what your insurance covers, please contact a member of our intake department at 734-222-4037.

What other services are available?

  • EHM Senior Solutions offers a wide range of services designed to keep seniors safe and independent in their own home. Adult daycare is designed to provide care and support for those suffering from memory loss while also providing a respite for their primary care giver. Our program supports the health, nutritional, social, and daily living needs of adults in a professionally staffed, group setting. Based on the Best Friends™ model, our personalized approach provides a setting infused with life-enriching activities for the benefit of both members and caregivers. We cater to the varied interests of our members and include them in daily planning to make the most out of their day with us. Each day brings a variety of fun things to do both indoors and out, offering plenty of opportunity for meaningful social interaction.

Participants engage in memorable experiences and focused activities that promote self-esteem and dignity. Caregivers know that while away from home their loved one is safe, active and engaged.

Medicare

Medicare Part A will pay for care in a nursing home for up to 100 days if the care that is required is considered “skilled”. In order to be considered “skilled,” nursing care must be ordered by a physician and delivered by, or under the supervision of, a professional such as a physical therapist, registered nurse or licensed practical nurse. Moreover, such care must be delivered on a daily basis. but there are a number of requirements that must be met before the nursing home stay will be covered. If you qualify, then Medicare may cover expenses related to your nursing home care in a skilled nursing facility for the first 100 days as follows: For days 1 to 20 of your stay during the benefit period, you pay $0. For days 21 to 100 of your stay, you pay a coinsurance that is set by the government.  Medicare generally doesn’t cover long-term care stays in a nursing home.

The key to understanding Medicare is that it provides reimbursement care for a specific time and is not intended to be utilized for long-term insurance coverage. The maximum benefit period is 100 days, but your medical condition may not qualify you to receive this length of coverage.

Medicare benefits apply under the following circumstances:

  • The patient spends three consecutive days in an acute care hospital, not including the day of discharge.
  • A physician certifies that the patient requires skilled or rehabilitative care.

If you have limited assets and a low income and you need help paying for nursing home or assisted living care, Medicaid might help you pay for your care. Medicaid will generally only cover the cost of “convalescent care” or what most people refer to as “long term care”. However, before Medicaid will pay for a nursing home stay, it must be proven “medically necessary” for the patient. The rules for Medicaid are complicated, but we can help. Our admissions professionals can help answer your questions and explain what Medicaid will and will not pay for.

All of our staff are trained in the Best Friends Approach™ to memory support. The Best Friends Approach starts with seven basic ‘building blocks’ that can help you learn to “see” persons with dementia differently and begin implementing a Best Friends Approach in your work:

Rehabilitation / Outpatient Therapy

Our Pathways TransitionCare® program incorporates a multi-disciplinary approach to aggressively pursue the goal of rehabilitation. We focus on restoring quality and purpose of daily living for those with a serious illness or injury. Pathways’ clinical specialists support our residents to maximize their recovery and independence. Our Therapy to Home Program® ensures a smooth and successful transition for our clients.

After an injury or illness all you want to do is get back to living your life as quickly as possible. Sometimes this means getting help from a trained professional who is there to work with you to build your strength, flexibility and muscle control. Our Pathways TransitionCare® program incorporates a multi-disciplinary approach to aggressively pursue the goal of rehabilitation. We focus on restoring quality and purpose of daily living for those with a serious illness or injury. Pathways’ clinical specialists support our residents to maximize their recovery and independence. Our Therapy to Home Program® ensures a smooth and successful transition for our clients. Our therapy treatment plans are overseen by a physician and may include:

Physical Therapy:  Physical therapy attempts to address the illnesses or injuries that limit a person’s ability to move and perform functional acts in their daily lives. PTs use an individual’s history and physical examination to arrive at a diagnosis and establish a management plan and, when necessary, incorporate the results of laboratory and imaging studies.

Occupational Therapy: A form of therapy for those recuperating from physical or mental illness that encourages rehabilitation through the performance of activities required in daily life.

Speech Therapy: Training to help people with speech and language problems. A speech therapist will address both the physical as well as cognitive impairments that may impact a person’s speech.

Aquatic Therapy: Treatments and exercises performed in water for relaxation, fitness, physical rehabilitation, and other therapeutic benefit. Typically a qualified aquatic therapist gives constant attendance to a person receiving treatment in a heated therapy pool.

Outpatient Therapy: Often times following an inpatient stay, additional services are required to extend and complement the treatment that was received during a hospital or nursing home stay. Other people may be referred directly from a physician to an outpatient program. In either case, our physical, occupational, and speech therapists can help you build your strength, endurance, flexibility and cognitive functioning to regain your independence.

 




Respite Services

Being a caregiver is an important role; however there are times in which additional support and help may be needed outside of the home. We offer many respite solutions for caregivers throughout our continuum of services. Whether respite is needed for a few days or several weeks, we can accommodate your needs. Our respite option gives caregivers the opportunity to rest and regain energy, with peace of mind knowing their loved one is being cared for, engaged, and active.

Being a caregiver for an aging or ill family member can be enormously rewarding, but it can also be exhausting and emotionally draining. As a caregiver, you feed, transport, clean, entertain, protect, and offer comfort and support. You manage your own busy schedule during the short, sometimes unpredictable lulls in your loved one’s schedule. It is no wonder that almost half of family caregivers rate their job as emotionally stressful.  Despite your very best intentions, caring for an elderly parent can often lead to caregiver burnout, depression, and health problems.

It is goal oriented treatment rendered immediately after, or instead of, acute hospitalization to treat one or more specific active complex medical conditions or to administer one or more technically complex treatments, in the context of a person’s underlying long-term conditions and overall situation.

Often referred to as Transitional Care, the focus of this type of care is to “transition” the patient as quickly as possible back to their previous level of functioning and independence.

Subacute care requires the coordinated services of an interdisciplinary team including physicians, nurses and other relevant professional disciplines, who are trained and knowledgeable to assess and manage these specific conditions and perform the necessary procedures. Subacute care is given as part of a specifically defined program.

The Redies Center for Rehabilitation and Healthy Living is the premier short term care facility designed to return those who are recovering from an injury or illness home as soon as possible. Our entire team of highly specialized physicians, nurses, therapists, wellness coordinators, dieticians, social workers, and support personnel all work with the patient and their loved ones to craft a care plan to emphasize independence and a transition home.Our goal is to help patient recover from an injury or illness and regain their independence as quickly as possible.  Our focus is to get you home.  We provide:

Medicare

Medicare Part A will pay for care in a nursing home for up to 100 days if the care that is required is considered “skilled”. In order to be considered “skilled,” nursing care must be ordered by a physician and delivered by, or under the supervision of, a professional such as a physical therapist, registered nurse or licensed practical nurse. Moreover, such care must be delivered on a daily basis. but there are a number of requirements that must be met before the nursing home stay will be covered. If you qualify, then Medicare may cover expenses related to your nursing home care in a skilled nursing facility for the first 100 days as follows: For days 1 to 20 of your stay during the benefit period, you pay $0. For days 21 to 100 of your stay, you pay a coinsurance that is set by the government.  Medicare generally doesn’t cover long-term care stays in a nursing home.

The key to understanding Medicare is that it provides reimbursement care for a specific time and is not intended to be utilized for long-term insurance coverage. The maximum benefit period is 100 days, but your medical condition may not qualify you to receive this length of coverage.

Medicare benefits apply under the following circumstances:

  • The patient spends three consecutive days in an acute care hospital, not including the day of discharge.
  • A physician certifies that the patient requires skilled or rehabilitative care.

Services typically covered by Medicare include:

  • Patient room
  • Meals, including therapeutic diets
  • Nursing care
  • Rehabilitation services
  • Medication
  • Medical supplies
  • Use of adaptive equipment and appliances
  • Ancillary services, such as laboratory tests and x-rays
